One of the key roles of the visiting pharmacist is to make sure that medicines are used safely and appropriately.

This involves monitoring the prescription charts to ensure legal requirements are met, and advising on potential adverse effects of medicines. The pharmacist will notify the appropriate clinical staff regarding any observations or interventions made, and will leave a written copy of these recommendations and comments. On the subsequent visit, the pharmacist checks if staff have taken appropriate action.

In order to ensure that the reported interventions are acted upon appropriately and in a timely manner, Ashtons has produced a pro-forma Standard Operating Procedure (SOP). This provides instructions for clinical staff on how to follow up the pharmacist’s visit, and recommends a method for reducing errors and improving quality.
