MHRA Improving medication error incident reporting and learning

NHS England and the MHRA set up the National Medication Safety Network last year. Their aim is to simplify and increase reporting, improve safety reports, maximise learning and improve practise to minimise harm from medication errors. Independent healthcare providers are being encouraged to take part.

Errors can be reported anonymously using a simple online form:

Hospitals can register to receive safety notices based on the error reporting by registering with the NHS Central Alerting System (CAS).

It is recommended that healthcare organisations appoint a Medication Safety Officer (MSO) who would be responsible for receiving safety notices and overseeing the error reporting.
